**Pilonidal sinus** (likely what you meant by "Piodinal sinus," a common misspelling) is a condition where a small tunnel or channel forms under the skin in the cleft at the top of the buttocks (near the tailbone). It often contains hair and debris, leading to the name "pilonidal" (from Latin for "nest of hair").

### Causes
It's generally an acquired condition, not congenital. Loose hairs in the area get drawn into the skin (often due to friction from sitting, tight clothing, or hormonal changes after puberty), causing inflammation and a foreign-body reaction. Risk factors include:
- Being male
- Young adulthood
- Excess body hair
- Obesity
- Prolonged sitting
- Family history
### Symptoms
Many people have no symptoms until it becomes infected:
- Pain, swelling, and redness in the area
- Pus or blood draining from a small opening
- Abscess formation (a painful lump)
- Foul-smelling discharge
- Low-grade fever (rare)
If uninfected, it may just appear as small pits or
dimples in the skin.
### Treatment
- **If asymptomatic**: No treatment needed; focus on hygiene (regular cleaning) and hair removal (shaving or laser) to prevent issues.
- **If infected (abscess)**: Incision and drainage under local anesthesia, often with antibiotics if there's spreading infection.
- **Recurrent or chronic cases**: Surgical options like excision of the sinus (wound left open to heal or closed with flaps for better results), or minimally invasive procedures (e.g., endoscopic cleaning).
- Recurrence is common, but good hygiene and hair control reduce risk.
See a doctor if you notice pain, swelling, or discharge in that area—early treatment prevents complications. Sources include NHS, Mayo Clinic, Cleveland Clinic, and medical reviews like StatPearls.
