Please share the original MRI films. My recommendation would be to see a Neurologist first and, if necessary (given the degree of damage on the MRI films), they will refer you to a Neurosurgeon. Most back pain can be managed without a procedure (operation) so I wouldn't jump to the surgical option.

no therapy he only said to hv belt n bed rest as she is not willing for surgery rest I'll share medication and she also facing pain in left leg til 2nd toe

I don't think surgery is the next best step here. I would advise against it.
Bed rest and belts is an old approach. Research has proven again and again that this is not the best way to manage back pain.
The pain radiating down to the 2nd toe is due to the involvement of the S1 nerve root.
Please share the medications and, if possible, ideally better to speak to the patient herself online and discuss options.
MRI films as well, please.

Just reviewed the prescription and MRI films.
Fortunately, the MRI is not too bad and I am pretty confident this can be managed without an operation.
The medications need optimization.
Please start her on the following...
1. Tab Synflex 500mg (One tablet in the morning and one at night) - For 7 days
2. Tab Hilin 100mg (One tablet at night ideally an hour before bed - Continous)
3. Tab Osam - D (One tablet in the morning and one at night - Continous)
Also, you can find good exercise regimen if you type L5-S1 Radiculopathy physiotherapy on Youtube. I would recommend she follows that.
If she follows the above plan, she will do very well.
I want to see her again in 1 week to see how she is doing and adjust treatment accordingly.
