Hello Doctor, I have been suffering from anal fissures for the past three years, ever since my C-section. Because of this, my life has become extremely difficult. I am unable to pass stool without taking laxatives, and I have become dependent on them. Even after taking laxatives, I experience severe pain while passing stool. I have used GTN and Xylocaine creams; they used to provide some relief earlier, but for the past 15–20 days there has been no improvement at all. The pain has become extremely severe.

Thanks for asking. Dear sister, a small superficial cut due to a hard stool, and weak pelvic muscles, or spasm of anal muscle due to pain after delivery, is the cause of the pain on passing stool. This starts a cycle of pain spasm and pain. One has to break this cycle to relieve the symptoms. Proper use of the analgesics breaks the cycle and fissure (cut) starts healing. On failure of medical treatment as your case is the most likely, one has to take the surgical option.
