Fatigue & Weakness Check
Tired by the middle of the afternoon, even after a full night’s sleep?
Tired by the middle of the afternoon, even after a full night's sleep? Hair falling more than it used to, legs heavy on the stairs, a mind that will not settle on one task? Most people put it down to work or age and reach for a multivitamin or an iron capsule. In Pakistan, four common and fixable things sit behind that feeling far more often than people expect: low vitamin D, low iron stores, low vitamin B12 and a slow thyroid. This check looks for all four, and for anaemia, from one blood sample taken at your home in Lahore.
They are worth testing together because they feel the same. Each one shows up as tiredness, weakness, low mood and poor concentration, and more than one is often low at the same time. They are also common: in Pakistan's National Nutrition Survey 2018, 79.7% of women of reproductive age were short of vitamin D, 41.7% were anaemic and 20.3% were short of vitamin B12. Guessing which one is low and treating it with a tablet from the medical store usually means treating the wrong one, or the right one at the wrong dose.
Your report is usually ready the same day. A PMC verified General Physician then goes through the five results with you, online or at a clinic, whichever suits you. The doctor tells you which result explains how you feel, what to take, at what dose and for how long, and when to test again so you know the treatment has worked. If everything is normal, that is useful too: it rules out the commonest causes and moves the conversation to the next question, such as sleep, blood sugar or mood.

Five tests from one blood sample, plus the doctor who reads them together.
- 25-OH Vitamin D3: the one most often low. A shortage causes tiredness, aching muscles and low mood, and it is the commonest deficiency in Pakistan. It is the result most worth knowing before you start a supplement, because the dose depends on how low you are.
- Serum Ferritin: your iron stores. Ferritin falls months before haemoglobin does, so it catches iron deficiency that a blood count alone would miss. Low ferritin is a common cause of tiredness and hair fall, especially in women.
- Complete Blood Examination (CBC): checks for anaemia and, from the size of your red cells, helps tell an iron problem from a B12 one. It also shows signs of infection, which matters because an infection can make ferritin read falsely normal.
- Vitamin B-12 (Serum): a shortage causes tiredness, pins and needles, poor memory and low mood. It is more likely if you eat little meat, or have taken metformin or acidity tablets for a long time.
- TSH: the first test for an underactive thyroid, which slows everything down: energy, weight, bowels, periods and mood. A normal TSH makes a thyroid cause unlikely.
- General Physician consultation, included: a PMC verified Marham doctor reads all five together and tells you what is low, what to take, at what dose and when to retest. You can take it online or at a clinic. It is part of the package, not an add on.
What this cannot do. It does not check blood sugar, so it cannot tell you whether tiredness after meals comes from rising sugar; for that, see the Sugar & Diabetes Check. It does not include free T3, free T4 or thyroid antibodies. It is not a sleep study, and it cannot diagnose depression or chronic fatigue syndrome. One result out of range confirms nothing on its own; your doctor reads it with your symptoms.
If something needs following up, your doctor orders only what is needed, charged separately: free thyroid tests, full iron studies, HbA1c or a specialist referral.

Not just a report. An answer.
Before your test
- No fasting needed. None of these five tests needs a fast. Eat and drink normally; water is always fine.
- Stop biotin 48 to 72 hours before the sample. Biotin is in many hair, skin and nail supplements and can give a false thyroid and B12 result.
- Tell us about any B12 injection, tablet or syrup, and any vitamin D injection, with the date. These keep the level looking normal for weeks or months, so the report can hide a real shortage. If you have not started them yet, test first.
- Mention iron capsules such as Sangobion if you already take them. They raise ferritin, so the result shows your treated level, not where you started.
- Do not stop any prescribed medicine for this test. Tell the doctor what you take instead.
- Postpone the test if you have a fever or an infection. Ferritin rises during any illness and can read falsely normal.
- Mention if you are pregnant, since the normal ranges for thyroid and iron are different then.
- Not for under 18s. For a tired child, book a Pediatrician consultation first.
- Have this ready for the consultation: how long you have felt this way, how you sleep, your periods if relevant, what you eat, and every medicine and supplement you take.
