What is the Serum Ferritin test?
A serum ferritin test measures how much iron your body is holding in storage. Ferritin is a protein that keeps iron locked inside your cells, and the amount circulating in your blood tracks closely with the size of those stores. That makes it the earliest reliable warning that iron is running low, often months before your haemoglobin drops far enough to register as anaemia on a routine blood count. At Marham Lab a serum ferritin test costs PKR 1,540 and your report is ready the same day.
Ferritin Test and Serum Ferritin: The Same Test
If your prescription says ferritin, S. Ferritin, ferritin level or serum ferritin, all four mean one blood test. MedlinePlus lists serum ferritin, serum ferritin level and ferritin serum as the standard alternative names for it, so there is nothing to compare or choose between. Pakistani lab forms usually shorten it to S. Ferritin, which is where most of the confusion starts.
One thing worth separating. A ferritin test is not the same as a serum iron test, even though people use the words interchangeably. Serum iron measures the iron moving through your bloodstream at that moment, which rises and falls with what you ate that morning. Ferritin measures the reserve. If your doctor wants to know whether you are running out of iron, ferritin is the one that answers the question.
What the Test Measures
Ferritin is a single measurement, but it is usually read next to the other iron markers. This is what each one adds.
| Marker | What it tells you |
|---|---|
| Ferritin | How much iron is held in storage across your body |
| Serum iron | How much iron is circulating right now, which swings with recent meals |
| TIBC (total iron binding capacity) | How much iron your blood has the capacity to carry |
| Transferrin saturation | What share of that carrying capacity is currently filled |
A doctor investigating tiredness or suspected anaemia will very often start with ferritin alone, because it is the marker that moves first.
Normal Range
The World Health Organization sets cut off values rather than a single normal band, because ferritin behaves differently by age and by whether inflammation is present. These figures come from the WHO guideline on use of ferritin concentrations to assess iron status.
| Group | Iron deficiency below | Iron overload risk above |
|---|---|---|
| Infants and children under 5 years | 12 µg/L | Not defined by WHO |
| Children 5 years and older, and adolescents | 15 µg/L | 150 µg/L in females, 200 µg/L in males |
| Adults 20 to 59 years | 15 µg/L | 150 µg/L in menstruating women, 200 µg/L in men and non menstruating women |
| Adults 60 years and above | 15 µg/L | Same as adults above |
| Pregnant women, first trimester | 15 µg/L | Not defined by WHO |
| Anyone with infection or inflammation | 30 µg/L in children, 70 µg/L in adults | Above 500 µg/L needs further evaluation |
Two cautions belong with that table. WHO states plainly that ferritin should never be used on its own to identify iron overload, and that anyone with a high result needs clinical and laboratory evaluation to establish why. And every laboratory calibrates its own assay, so the reference interval printed on your own report is the one that applies to your result.
What High and Low Values Mean
| Result | What it can mean | What usually happens next |
|---|---|---|
| Low ferritin | Your iron stores are depleted. This is the earliest stage of iron deficiency and can progress to iron deficiency anaemia if the cause continues | Your doctor looks for where the iron is going, such as heavy periods or poor absorption, rather than only replacing it |
| Normal ferritin but symptoms continue | Infection, recent surgery or ongoing inflammation raises ferritin and can hide a real deficiency. This is why WHO lifts the adult cut off to 70 µg/L in that situation | A CRP test alongside ferritin helps show whether inflammation is distorting the number |
| High ferritin | Inflammation, infection, liver disease, obesity, alcohol use disorder, hyperthyroidism or cancer can all raise it, according to MedlinePlus. Genuine iron overload is only one of several explanations | Further tests to find the underlying cause, never a diagnosis from ferritin alone |
One result outside the range confirms nothing on its own. Take your report to a doctor who can read it against your symptoms and history.
Who Should Get Tested and When
Early iron deficiency has no symptoms at all. Your body quietly draws down its reserve first, and you only start feeling it once the stores are nearly gone. That is exactly why testing is based on risk rather than on waiting to feel unwell.
Consider a ferritin test if any of these apply to you.
- You have heavy or prolonged menstrual bleeding
- You are pregnant, or planning a pregnancy
- You feel persistently tired, breathless, dizzy or look pale
- You have restless legs, a symptom MedlinePlus links directly to low iron
- You have inflammatory bowel disease, coeliac disease, or have had gastrointestinal surgery that affects absorption
- Your haemoglobin or MCH came back low on a recent blood count
- You are being treated with iron and your doctor wants to check whether it is working
The background matters in Pakistan. WHO reports that iron deficiency is the main cause of anaemia, which affects 33% of non pregnant women, 40% of pregnant women and 42% of children worldwide. Locally the picture is heavier still, with the National Nutrition Survey 2018 finding 43% of women of reproductive age anaemic.
How to Prepare
MedlinePlus notes that your provider may ask you to fast for 12 hours before the test, and that it is usually done in the morning. Book a morning slot and treat the fast as the default unless your doctor tells you otherwise, since arriving unprepared can mean repeating the visit. Water is fine.
Tell the phlebotomist if you are taking iron supplements or a multivitamin containing iron, because these lift your result and change how it should be read. Also mention any recent illness, infection or surgery, for the same reason in the other direction.
What Happens During the Test
A trained phlebotomist takes a small blood sample from a vein in your arm using a fine needle. The whole draw takes under five minutes. You may feel a brief sting as the needle goes in, and a little bruising afterwards is normal and settles quickly. In Lahore you can have this done at home instead of travelling to a collection point.
Conditions It Can Help Detect
Ferritin is ordered most often to confirm or rule out iron deficiency anaemia, the commonest nutritional deficiency worldwide. It also helps investigate haemochromatosis, a condition in which the body absorbs and stores too much iron, and it is used to monitor iron status in long term conditions such as chronic kidney disease, cancer and autoimmune disease. Because most of your body's ferritin is stored in the liver, liver disease frequently shows up as a raised result. MedlinePlus also links low ferritin to restless legs syndrome, and unusually high ferritin to adult Still disease.
What ferritin cannot do is stand alone. It tells you how much iron is in the tank. It does not tell you why the tank is emptying, and that question is the one that actually changes your treatment.

Final Thoughts
Ferritin is the test that catches iron deficiency early, while it is still easy to correct and before it becomes anaemia. That is its value, and it is why doctors reach for it ahead of the other iron markers. Book it when you carry one of the risk factors above rather than waiting until tiredness becomes hard to ignore, and take the report to a doctor who can read the number against everything else going on with you.
