What is the Serum Ferritin test?
Ferritin Test and Serum Ferritin: The Same Test
If your prescription says ferritin, S. Ferritin, ferritin level or serum ferritin, all four mean one blood test. MedlinePlus lists serum ferritin, serum ferritin level and ferritin serum as the standard alternative names for it, so there is nothing to compare or choose between. Pakistani lab forms usually shorten it to S. Ferritin, which is where most of the confusion starts.
One thing worth separating. A ferritin test is not the same as a serum iron test, even though people use the words interchangeably. Serum iron measures the iron moving through your bloodstream at that moment, which rises and falls with what you ate that morning. Ferritin measures the reserve. If your doctor wants to know whether you are running out of iron, ferritin is the one that answers the question.
What the Test Measures
Ferritin is a single measurement, but it is usually read next to the other iron markers. This is what each one adds.

Normal Range
The World Health Organization sets cut off values rather than a single normal band, because ferritin behaves differently by age and by whether inflammation is present. These figures come from the WHO guideline on use of ferritin concentrations to assess iron status.

Two cautions belong with that table. WHO states plainly that ferritin should never be used on its own to identify iron overload, and that anyone with a high result needs clinical and laboratory evaluation to establish why. And every laboratory calibrates its own assay, so the reference interval printed on your own report is the one that applies to your result.
What High and Low Values Mean

Who Should Get Tested and When
Early iron deficiency has no symptoms at all. Your body quietly draws down its reserve first, and you only start feeling it once the stores are nearly gone. That is exactly why testing is based on risk rather than on waiting to feel unwell.
