my first son is autistic 2020 born 2nd baby was spina bifida baby girl 2023 born she had surgeries and after a year she passed away its traumatising i wanted to concieve a healthy baby please help me drs my blood group is A- and yes its a cousin marriage

No one can guarantee for that. Believe on almighty Allah for best. For prevention of spina bifida start dolic acid tablets one daily one month before conceiving and continue till delivery of baby.
I'd recommend prenatal genetic testing.

Start taking folic acid 5 mg daily ideally 3 months before conception and continue for 3 months during pregnancy. It will help to reduce the chance of spina bifida in newborn baby. May Allah bless you and ease your pain.
