What is the HbA1C test?
HbA1c is a blood test that shows your average blood sugar over the past 3 months in a single number. A finger prick sugar reading tells you what your sugar is doing this minute. HbA1c tells you what it has been doing since roughly the start of the season.
It works because glucose sticks to the haemoglobin inside your red blood cells. The National Institute of Diabetes and Digestive and Kidney Diseases explains that the more glucose there is in your blood, the more of it attaches, and the test measures how much haemoglobin is carrying glucose. The result comes back as a percentage.

What the Test Measures
| What it shows | What it does not show |
|---|---|
| Your average blood sugar across about 3 months | What your sugar is right now |
| Whether your sugar has been controlled over time | A single high reading after a heavy meal |
| Whether diabetes treatment is working | Sudden short lived spikes or drops |
Because it averages, one bad week does not ruin the number, and one good day before the test does not fix it. That is the point of it.
HbA1c Normal Range
NIDDK sets out the diagnostic bands as follows. Any result used to diagnose diabetes needs a second confirming test unless the symptoms are already clear.
| HbA1c level | What it means |
|---|---|
| Below 5.7 percent | Normal |
| 5.7 to 6.4 percent | Prediabetes |
| 6.5 percent or above | Diabetes |
Within the prediabetes band, the higher the number, the greater the chance of going on to develop type 2 diabetes. If you already have diabetes, your doctor sets a personal target. Many people aim below 7 percent, but NIDDK notes that a level which is safe for one person may not be safe for another.
What High and Low Values Mean
An HbA1c result can be misleading in ways that matter a lot in Pakistan.
| Situation | Effect on the result |
|---|---|
| Iron deficiency anaemia | Can push HbA1c falsely high |
| Recent blood loss or transfusion | Changes the result, because it changes red cell life span |
| Kidney failure or liver disease | Can give a false reading |
| Haemoglobin variants | Can interfere with some HbA1c methods |
That first row deserves attention here. Anaemia is widespread in Pakistan, with the National Nutrition Survey 2018 finding 43 percent of women of reproductive age anaemic, so a falsely high HbA1c is not a rare edge case. NIDDK also notes that people of South Asian descent may carry a haemoglobin variant that interferes with some HbA1c tests, often without knowing it.
If your HbA1c and your sugar readings do not agree, tell your doctor rather than trusting one over the other.
Who Should Get Tested and When
- If you have diabetes, at least twice a year, and more often if you are not meeting your target
- If you have prediabetes, on the schedule your doctor sets
- If you have risk factors for type 2 diabetes, ask your doctor whether you should be tested
- If you are pregnant, note that the glucose challenge or tolerance test is used for gestational diabetes, not HbA1c
Test sooner if you are unusually thirsty, passing urine more than usual, losing weight without trying, feeling very tired, or healing slowly from cuts.
How to Prepare
- No fasting is needed. NIDDK is explicit that you do not have to fast for an A1C test
- You can give the sample at any time of day
- Eat and take your medicines as normal
- Tell your doctor if you have anaemia, a blood disorder, or have had a transfusion recently
- If your doctor has also ordered a fasting sugar or a lipid profile, fast for those instead
What Happens During the Test
- A trained phlebotomist draws a small blood sample from a vein in your arm
- The draw takes under a minute
- You may feel a brief sting and see a small bruise afterwards
- You can eat and drink normally straight away
- Your Marham Lab report is ready the same day and reaches you online
