What is the RFTs (Renal Function Test with eGFR) test?

RFT stands for renal function test. It is a blood test that shows how well your kidneys are clearing waste from your blood. One sample measures urea, creatinine, uric acid and electrolytes, and it calculates your eGFR, the single number that says how well your kidneys are filtering.

Kidneys fail quietly. The National Institute of Diabetes and Digestive and Kidney Diseases puts it plainly: early kidney disease usually has no symptoms, and testing is the only way to know how well your kidneys are working.

KFT, Kidney Function Test and RFT: The Same Test

If your slip says KFT, RFT, RFTs, kidney function test or renal function test, it is one test. KFT stands for kidney function test and RFT for renal function test. Renal simply means relating to the kidneys, so the two abbreviations describe the same panel and Pakistani labs use both, sometimes on the same report.

You may also see it written as urea creatinine, or as a kidney profile. If a doctor asked for any of these, this is the test.

What the Test Measures

Each marker looks at a different part of kidney function.

MarkerWhat it isWhy it matters
eGFREstimated glomerular filtration rateThe headline number, showing how fast your kidneys filter blood
CreatinineWaste from normal muscle breakdownYour kidneys clear it, so it rises as kidney function falls, and it is what eGFR is calculated from
UreaWaste from protein breakdownRises when the kidneys are not clearing well, but diet and dehydration move it too
Uric acidWaste from the breakdown of purinesLinks to gout and kidney stones as well as kidney function
Sodium and potassiumElectrolytes the kidneys balanceGo out of range when the kidneys cannot regulate fluid and salts
Chloride and bicarbonateThe rest of the electrolyte pictureHelp show acid and base balance

What Your eGFR Number Means

eGFR is the number your doctor looks at first. NIDDK sets out the bands as follows.

eGFRWhat it means
60 or aboveWithin the normal range
Below 60May mean kidney disease, and needs a conversation with your doctor
15 or belowKidney failure, where most people need dialysis or a transplant

You cannot raise your eGFR, but you can work to stop it falling further. That is why finding it early matters so much.

What High and Low Values Mean

Reference ranges for urea, creatinine and uric acid differ between laboratories and between testing methods, and they shift with age, sex and muscle mass. Read the range printed on your own report rather than comparing against a number you found online.

FindingWhat it may suggest
High creatinine with low eGFRThe kidneys are filtering less well than they should
High urea with normal creatinineOften dehydration, a high protein diet or bleeding in the gut rather than kidney damage
High uric acidLinked to gout and kidney stones, and common alongside diabetes and obesity
Potassium out of rangeNeeds prompt medical attention, because it affects the heart
Low creatinineUsually harmless and often just low muscle mass

One value outside the range confirms nothing. Your doctor reads the whole panel together with your blood pressure, sugar and history.

Who Should Get Tested and When

Because early kidney disease is silent, testing is based on risk rather than symptoms. NIDDK advises getting checked if you have any of the following.

  • Diabetes, and in that case every year
  • High blood pressure
  • Heart disease
  • A family history of kidney failure

Test sooner if you have swelling in the feet or ankles, pass much more or much less urine than usual, have foamy urine, feel persistently tired, or take medicines that put a load on the kidneys.

How to Prepare

  • No fasting is needed for the RFT on its own
  • Drink water normally, since being dehydrated can lift your urea reading
  • Tell your doctor about every medicine and supplement you take
  • If your doctor has bundled the RFT with a lipid profile or a fasting sugar, fast for those instead
  • Avoid heavy exercise the day before, because it can raise creatinine briefly

What Happens During the Test

  • A trained phlebotomist draws a small blood sample from a vein in your arm
  • The draw takes under a minute
  • You may feel a brief sting and see a small bruise afterwards
  • You can eat and drink normally straight away
  • Your Marham Lab report is ready the same day and reaches you online

Conditions It Can Help Detect

An RFT is a screening and monitoring test rather than a diagnosis on its own. It helps find chronic kidney disease early, and it is used to follow kidney health in people with diabetes or high blood pressure, to check the effect of medicines on the kidneys, and to look into stones, gout and electrolyte problems.

Your doctor may pair it with a urine albumin test. NIDDK explains that a healthy kidney does not let albumin pass into urine, and that on the urine albumin to creatinine ratio a result of 30 mg/g or less is normal while more than 30 mg/g may be a sign of kidney disease. Blood and urine together give a fuller picture than either alone.

Final Thoughts

Kidney damage gives no warning until a lot of function is already gone, and an RFT is the cheapest way to see it coming. If you live with diabetes or high blood pressure, this is a test to repeat on the schedule your doctor sets rather than only when something feels wrong. Take the report to a doctor, because eGFR read alongside your other results says far more than any single number on the page.

Frequently asked questions

Yes. RFT stands for renal function test and KFT for kidney function test. Renal means relating to the kidneys, so both abbreviations describe the same blood panel, and Pakistani labs use them interchangeably. If your slip says RFT, RFTs, KFT, kidney function test or renal function test, this is the test your doctor wants.

RFT stands for renal function test. Renal refers to the kidneys, so a renal function test is simply a test of how well your kidneys are working. The same panel is written as KFT, meaning kidney function test, and both appear on lab slips in Pakistan. The full form matters only because it tells you the test is about your kidneys, not about anything else.

It is included. The Marham Lab panel is listed as RFTs with eGFR, so the filtration number comes with the test at PKR 1,260. Booking eGFR on its own costs the same PKR 1,260 and gives you far less, so there is no reason to order it separately unless your doctor has asked for that one value.

At Marham Lab the full RFT panel with eGFR is PKR 1,260, reduced from PKR 1,800, and the rate is the same in every city. Single markers cost PKR 560 each for urea, creatinine or uric acid, and PKR 1,050 for the electrolyte panel, so the full test is better value than picking two or three. Home sample collection is currently available in Lahore only.
