If you are planning a baby, the useful time to find a problem is before you are pregnant, not at the twelve week scan. Five things are worth knowing in advance, and all five are fixable or manageable if you know about them early: whether you are anaemic, your blood group and Rh factor, whether your thyroid is working properly, whether your blood sugar is already creeping up, and your vitamin D level. The Planning a Baby Check measures all five from one blood sample taken at your home in Lahore, and a Gynaecologist goes through the results with you.

Each one earns its place. Anaemia is very common here, at 41.7% of women of reproductive age (NNS 2018, UNICEF), and pregnancy makes it worse, so correcting it first is far easier than treating it later. Blood group and Rh factor need to be on record before any pregnancy, because an Rh negative mother may need an anti D injection at the right time. An underactive thyroid makes conception harder and raises miscarriage risk, and it is treated with a simple daily tablet. Sugar that is already high is safer to bring down before conception than during. Vitamin D deficiency runs at 79.7% of women of reproductive age in the same survey.

What's included

Five tests from one blood sample, plus the doctor who reads them with you.

  • Complete Blood Examination (CBC): shows whether you are anaemic before pregnancy adds to the demand on your body. Low haemoglobin going into pregnancy is linked to tiredness, a harder delivery and a smaller baby, and it is usually correctable with iron over a few months.
  • Blood Group & Rh Factor: needed on record before any pregnancy. If you are Rh negative and your husband is Rh positive, the baby can be Rh positive too, and you may need an anti D injection at the right point to protect future pregnancies.
  • TSH: the thyroid check. An underactive thyroid makes conceiving harder and raises the risk of miscarriage, yet it often has no obvious symptoms and is treated with one tablet a day.
  • Blood Sugar Fasting (BSF): sugar that is already high before pregnancy is a different and more serious problem than gestational diabetes, and it is much safer to bring down beforehand.
  • 25-OH Vitamin D3: deficiency is very common in Pakistani women and is linked to weaker bones in the baby, so it is worth correcting before rather than during.

If something needs following up, your doctor orders only what is needed, charged separately: serum ferritin if the CBC shows anaemia, free T4 if the TSH is off, rubella immunity, or thalassaemia screening for both partners.

Before your test

  • Fasting needed: nothing to eat for 8 to 12 hours before the sample, because of the fasting sugar reading. Plain water is fine. Book a morning slot so you fast overnight and can eat straight after.
  • Do not book during your period. Bleeding lowers haemoglobin temporarily and can make the CBC read as anaemia when there is none. Wait until a few days after it finishes.
  • Stop vitamin supplements 24 hours before the sample. Multivitamins and vitamin D sachets raise the vitamin D reading and can hide a real deficiency. If you are already on folic acid, keep taking it and tell the doctor.
  • If you have had a vitamin D injection in the last few months, say so. The level stays raised for a long time afterwards and the result will not reflect your baseline.
  • Tell the doctor about thyroid medicine, metformin, steroids and any fertility treatment you are already on, since all of them change these results.
  • If you think you may already be pregnant, say so before the sample is taken. Pregnancy changes what a normal result means for every test in this panel, and your doctor will read it differently.
  • Have your husband's blood group to hand if you know it. It matters for the Rh discussion.
  • Not for children under 12.
  • Have a phone in a quiet place for the video consultation, since this speciality is online only.

Before. Folic acid protects the baby's developing spine and brain, and that development happens in the first few weeks, often before a woman knows she is pregnant. Waiting for a positive test usually means starting too late to get the full benefit, which is why it is normally advised for a few months before you begin trying. The dose is not the same for everyone: women with diabetes, epilepsy, a high BMI or a previous affected pregnancy are usually advised a much higher dose, which is a decision for your doctor rather than a pharmacy counter. Keep taking it on the day of the test and mention it in the consultation.
