Your period is three weeks late. Or it came twice this month. Or it has stopped altogether, or it arrives so heavy that you plan the week around it. The usual next step is a tablet from the medical store to bring it on or hold it back, and that does produce a bleed. What it does not do is answer why the cycle went wrong, and several of the reasons get quietly worse while you wait. The Irregular Periods Check measures six things that decide how a cycle runs, from one blood sample taken at your home in Lahore, and a Gynaecologist reads them with you afterwards.

The six answer three separate questions. Whether your thyroid is behind it: TSH, because an underactive or overactive thyroid is one of the commonest and most treatable causes of periods that stop, come late or turn heavy, and it is easy to miss because the other symptoms get blamed on something else. Whether the bleeding has cost you: haemoglobin and ferritin together, which show both whether you are anaemic and whether your iron stores are running down, since stores fall months before haemoglobin does. And what the ovaries are doing: LH and FSH, the two pituitary hormones that drive ovulation, read against the day of your cycle. Vitamin D is included because deficiency affects roughly four in five Pakistani women of reproductive age, and it is a common reason for the fatigue that arrives alongside.

Your report is usually ready the same day. A PMC verified Gynaecologist then reads all six with you online. Timing matters as much as the numbers here, so read the precautions before you book: LH and FSH only mean something when the sample is taken in the first few days of a cycle. If the thyroid is the problem, treating it usually settles the cycle on its own. If the picture points towards PCOS, blood tests cannot confirm that by themselves and your doctor will tell you what else is needed. And if the bleeding is heavy enough to be causing the anaemia, that is a problem in its own right and worth treating rather than tolerating.

What's included

  • TSH (Thyroid Stimulating Hormone): the first test of thyroid function and the single most useful one on this list. An underactive thyroid can make periods heavier, longer or more frequent and can eventually stop them; an overactive one usually makes them lighter or makes them disappear. It is treatable, and treating it often settles the cycle by itself.
  • Hb (Haemoglobin): whether heavy or frequent bleeding has left you anaemic. This is very often the real reason behind breathlessness on stairs and the tiredness that gets written off as weakness.
  • Serum Ferritin: your iron stores. Ferritin falls months before haemoglobin does, so a normal Hb sitting next to a low ferritin is a common and important result: the iron is running out and the anaemia simply has not arrived yet.
  • 25-OH Vitamin D3: deficiency affects roughly four in five women of reproductive age in Pakistan and is a frequent contributor to the fatigue, low mood and body aches that turn up alongside cycle problems.
  • LH (Luteinising Hormone) and FSH (Follicle Stimulating Hormone): the two pituitary hormones that drive ovulation. Read together, and against the day of your cycle, they show whether the ovaries are being signalled properly. They point towards the group of conditions that includes PCOS, and a high FSH points somewhere different again, towards ovaries that are running out of reserve earlier than expected.
  • Gynaecologist consultation, included: a Marham Gynaecologist reads all six against your cycle history and tells you the likely cause and what to do next. This consultation is online only, which for most women is the point, since you can take it from home. It is part of the package, not an add on.

What this package cannot do. It cannot confirm PCOS on its own. That diagnosis needs your symptoms and usually an ultrasound as well, and your doctor will arrange those if they are warranted. There is no pregnancy test, no ultrasound and no internal examination anywhere in this package.

If something needs following up, your doctor orders only what is needed, charged separately: prolactin, testosterone, a fasting insulin or HbA1c, a full blood count if the anaemia needs typing, or a pelvic ultrasound.

Before your test

  • Timing is critical. LH and FSH should be taken on day 2, 3, 4 or 5 of your cycle, counting the first day of proper bleeding as day 1. Taken at another point they are still a real measurement, but there is no reference range to read them against and the report becomes far harder to interpret. Book your sample for those days.
  • If your periods have stopped altogether, do not wait for one in order to test. Say so when you book, give the sample whenever suits you, and your doctor will read the result knowing there was no cycle to time it to.
  • Rule out pregnancy first. A missed period is far more often a pregnancy than a hormone problem. A home urine pregnancy test costs very little and should come before this package if there is any possibility at all.
  • Fasting is not required. Eat and drink normally before the sample.
  • Skip iron tablets and iron rich supplements for 24 hours before the sample. A dose taken that morning can lift the ferritin reading and hide a deficiency that is really there.
  • Tell the doctor about every hormone tablet you take or have recently taken, including contraceptive pills and anything the pharmacy gave you to bring on or delay a period. They change LH, FSH and the whole shape of the report, and how recently you took them matters.
  • Ferritin also rises during any infection or fever, so postpone the sample if you are unwell. A result taken during illness can look normal while your stores are actually low.
  • Have your dates ready for the consultation: when the last period started, how long your cycles usually run, how many days you bleed and whether you pass clots. These matter as much as the numbers do.
  • Not for under 18s. For a daughter under 18 with the same problem, book a consultation first rather than a panel, because the reference ranges and the likely reasons are different in teenagers.
  • Bleeding that soaks a pad an hour, bleeding with severe pain or dizziness, or any bleeding after menopause is not something to test and wait on. Get seen the same day.
