Hepatitis B and C do not announce themselves. Most people who carry either one feel completely well for years, sometimes decades, while the liver is quietly damaged, and by the time symptoms arrive the treatable window has narrowed. Pakistan carries one of the largest hepatitis C burdens in the world, and the ways it spreads here are ordinary rather than exotic: a reused syringe, a dental instrument, a barber's razor, an old transfusion. This check screens for both viruses and puts a full first health panel around them, from one blood sample and one urine sample collected at your home in Lahore.

Two of the nine answer the hepatitis question: HBsAg for hepatitis B and Anti HCV for hepatitis C. The other seven are the baseline every adult should have on record. A blood count with film review for anaemia and infection. Fasting blood sugar. A lipid profile for cholesterol. LFTs, which matter twice over here, since raised liver enzymes alongside a positive viral test change what happens next. RFTs with eGFR for the kidneys, TSH for the thyroid, and a urine examination, which catches protein, sugar, blood and infection that nothing in the blood tube would show.

One thing to understand before you book: these two are screening tests, not diagnoses. A reactive Anti HCV means the virus has been in your body at some point, not that it is there now, and confirming active infection needs a separate PCR test your doctor will order. That distinction matters, because hepatitis C is now curable in most people with a course of tablets, and knowing which situation you are in is the whole point of testing.

What's included

Nine tests from one blood sample and one urine sample, plus the doctor who reads them together.

  • HBsAg: the standard screening test for hepatitis B. It looks for a protein on the surface of the virus, so a positive result means the virus is present and needs following up, not that you were merely exposed once.
  • Anti HCV: the standard screening test for hepatitis C. It looks for your antibodies rather than the virus itself, which is why a reactive result is a reason to test further, not a diagnosis. Antibodies stay positive for life, including in people who cleared the infection on their own or were cured years ago. Your doctor confirms active infection with an HCV RNA PCR test, ordered separately.
  • Blood C/E (CBC with blood film review): haemoglobin, white cells and platelets, plus a technologist looking at the cells under a microscope. The film matters here, because a low platelet count is one of the earliest signs that a liver has been quietly scarred.
  • LFTs: bilirubin, ALT, AST, ALP, GGT, albumin and total protein. Read alongside the viral tests these change the plan: a positive result with normal enzymes is a different situation from a positive result with the liver already inflamed.
  • Blood Sugar Fasting: your sugar level after an overnight fast. A snapshot rather than an average. If diabetes is your main concern rather than hepatitis, an HbA1c is the better test and covers three months instead of one morning.
  • Lipid Profile: total cholesterol, LDL, HDL and triglycerides.
  • RFTs with eGFR: urea, creatinine, electrolytes and an estimated filtration rate. Kidney function also decides which hepatitis treatments are suitable, so it is not filler here.
  • TSH: the first test of thyroid function.
  • Urine for C/E: protein, sugar, blood, and signs of infection. The one test here that finds things no blood sample would.

What this does not include. There is no HCV RNA PCR, no hepatitis B viral load, no anti-HBs to tell you whether your vaccination worked, no ultrasound and no fibrosis scan. Those are the next steps when a screen comes back positive, and your doctor orders only the ones you need.

Before your test

  • Fast for 8 to 12 hours before the sample. Plain water is fine. The fast is for the lipid profile and the fasting sugar; the hepatitis tests, the blood count, the liver, kidney and thyroid tests are unaffected by food.
  • Book a morning slot, which is the practical end of an overnight fast.
  • Collect the urine correctly, because this is the one sample you handle yourself. Use the first urine of the morning if you can, wash your hands, pass a little into the toilet first and catch the middle of the stream in the container. A carelessly taken sample is the commonest reason a urine report has to be repeated.
  • Women should not give a urine sample during a period. Blood in the sample makes the result impossible to read properly. Reschedule for a few days after it finishes.
  • A hepatitis B vaccination does not make HBsAg positive, so being vaccinated is not a reason to skip this test. If what you actually want to know is whether your vaccination worked, that is a different test called anti-HBs, and your doctor can order it.
  • Tell the doctor about every herbal or hakeem preparation and any regular paracetamol. Both raise liver enzymes on their own, and an unexplained raised ALT is exactly the finding this panel is trying to interpret correctly.
  • Avoid alcohol for at least 72 hours before, if it applies to you, since it moves both the liver enzymes and the triglycerides.
  • Do not train hard in the 48 hours before the sample. Heavy exercise raises AST and creatinine and can make a healthy liver and healthy kidneys look abnormal.
  • A negative result does not cover a very recent exposure. Both tests need weeks to turn positive after an infection, so if you have a specific worry about something that happened in the last month or two, say so, and your doctor will tell you when to repeat it.
  • Postpone the panel if you have a fever or an active infection.
  • Not for under 18s.
  • Your results are yours. A positive screen is a medical finding to be worked up, not a verdict, and the consultation is a private one to one conversation with the doctor.

Because feeling fine is exactly what hepatitis B and C do for most of the time they are present. Both can sit in the liver for years or decades without producing a single symptom, and the damage they cause accumulates quietly during that period. That is why testing is done on risk rather than on symptoms, and in Pakistan the risk is not unusual: a dental procedure, an injection given with a reused syringe, a shave at a barber, a transfusion before screening became routine, or a household member who is already positive. The other reason is that the answer changes what you can do. Hepatitis C is curable in most people now, hepatitis B can be controlled and vaccinated against, and both are far easier to deal with before the liver is scarred than after.

They share five tests and exist for two different reasons, so it is worth picking rather than buying both. The Fatty Liver & Cholesterol Check is the yearly check for someone with no particular worry: it uses HbA1c, which gives your average sugar over three months, and it is built around the silent conditions that come from diet and a desk job. This one is built around the hepatitis question, adds a urine examination and a blood film, and uses a fasting sugar rather than an HbA1c.
