Feeling Unwell Check
Some days you just do not feel right.
Some days you just do not feel right. Low energy, getting winded on the stairs, catching every bug going around, headaches, or a heaviness you cannot put your finger on. There is no single symptom to point at, so most people wait it out. The Feeling Unwell Check is the cheapest way to stop guessing: three blood tests from one sample taken at your home in Lahore, covering the three ordinary things that most often explain feeling below par in Pakistan, khoon ki kami (anaemia), a hidden infection or inflammation, and high blood sugar.
Each test answers a different question. The CBC looks at your red cells, white cells and platelets, so it picks up anaemia and shows when your body is fighting an infection. The ESR is a simple inflammation marker that rises when something is quietly inflamed, which a CBC alone can miss. The random blood sugar reading flags sugar that has drifted high. Anaemia in particular is common here: the National Nutrition Survey found 41.7% of women of reproductive age anaemic (NNS 2018, UNICEF). This is also the check to run once a year when nothing is wrong, so you have a baseline to compare against later.
Your report is usually ready the same day. A PMC verified General Physician then reads it with you, online or at a clinic, whichever suits you, in Urdu or English. If everything comes back normal, that is a real answer: anaemia, active infection and high sugar are ruled out, and the doctor can look at sleep, stress, thyroid or vitamin D next. If something is off, they tell you what it means and what to do about it, and order the deeper test if you need one.

- Complete Blood Examination (CBC): your full blood picture. Haemoglobin and red cell counts show whether you are anaemic; white cells rise when your body is fighting an infection; platelets matter for bleeding and for dengue season. This is the single highest yield test when you feel unwell without knowing why.
- ESR (Erythrocyte Sedimentation Rate): a simple, sensitive marker of inflammation. It rises when something is quietly inflamed or infected somewhere in the body, which a CBC on its own can miss. It tells your doctor whether to keep looking.
- Blood Sugar Random (BSR): your blood sugar right now, whenever the sample is taken. It is a quick screen for sugar that has drifted high. It flags a problem rather than confirming one, so a high reading is followed by a fasting sugar or HbA1c.
- General Physician consultation, included: a Marham doctor goes through all three results with you, tells you which of the three causes is in play, and starts treatment or orders the next test. Take it online or at a clinic, whichever suits you. It is part of the package, not an add on.
If a result points somewhere specific, your doctor may suggest a follow up, charged only if you need it: serum ferritin and vitamin B12 for anaemia, HbA1c for sugar, or a urine test for infection.

If everything is normal, that is useful too: it rules out the commonest causes and moves the conversation to the next question, such as sleep, blood sugar or mood.
Not just a report. An answer.
Before your test
- No fasting needed. Eat and drink as normal. A morning sample is best, and the phlebotomist collects it at your home.
- The sugar reading here is a random one, so note roughly when you last ate and tell the doctor. A high random reading does not confirm diabetes on its own; it points to a fasting sugar or HbA1c next.
- If you have had a fever, a cold or any infection in the last two weeks, tell the doctor or wait a week. ESR and white cell counts rise during and after an illness and can read high for reasons that have nothing to do with what you came for.
- Keep taking your regular medicines, but mention iron tablets, vitamin supplements, steroids or anti inflammatory painkillers. All of them can shift these results.
- Women: mention if you are pregnant or currently on your period. ESR runs higher in pregnancy, and a period can lower haemoglobin.
- Not for children under 12.
- Have the patient's name and age ready for the report, and either a phone in a quiet place for an online consultation or your preferred clinic if you would rather be seen in person.
- One out of range value confirms nothing on its own. Your doctor will read the full report with you before anything is decided.

خون کی کمی (anaemia) کی علامات کیا ہیں اور کون سا ٹیسٹ بتاتا ہے؟

CBC ٹیسٹ کیا بتاتا ہے؟

Is a random sugar test enough to tell me if I have diabetes?
No, and it is not meant to be. The random sugar reading in this check tells you your blood sugar at the moment the sample is taken, whatever you last ate. It is a fast, cheap screen: a normal reading is reassuring, and a high one is a flag. Confirming diabetes needs a fasting sugar reading or an HbA1c, which shows your average over the last two to three months, and usually a repeat test as well. If your reading comes back high, your doctor will order the right follow up in the same consultation.
