What is the MCH test?
Most people arrive at this page holding a CBC report with a line on it they do not recognise. MCH stands for mean corpuscular haemoglobin, and it is the average amount of haemoglobin carried inside each of your red blood cells.
Haemoglobin is what moves oxygen around your body. MCH tells your doctor how much of it each cell is carrying, which is a different question from how many cells you have.
MCH Is Part of the CBC, Not a Separate Test
This is the first thing worth knowing, and it can save you money.
MCH is one of the red blood cell indices reported inside a complete blood count, alongside MCV and MCHC. MedlinePlus describes these indices as part of the CBC, used together to work out which type of anaemia a person has.

A low MCV alongside a low MCH widens the picture further, since MedlinePlus links a low MCV to low iron, lead poisoning or thalassaemia. That last one matters in Pakistan, where thalassaemia carrier rates are high enough that a persistently low MCH in someone whose iron is normal is worth investigating properly rather than treating with iron tablets.
Why Low MCH Is So Common Here
Iron deficiency is the usual explanation, and it is widespread. The National Nutrition Survey 2018 found 43 percent of women of reproductive age in Pakistan were anaemic, and the World Health Organization notes that anaemia is especially common in women and children.
That is why a low MCH is rarely a surprise on a Pakistani report. It is also why it should not be waved through. The number tells you the shape of the problem, not the cause, and iron tablets are not the right answer for every low reading.
When Your Doctor Will Look at MCH
- You have symptoms of anaemia such as tiredness, breathlessness, pale skin or a racing heartbeat
- Your haemoglobin came back low and the cause is not yet clear
- You are being followed up for iron deficiency, B12 deficiency or folate deficiency
- You have heavy periods, a restricted diet, or a condition that affects absorption
- There is thalassaemia in your family
- You are on chemotherapy or a medicine that affects the bone marrow

- Eat and drink normally before the sample
- Tell your doctor if you have started iron, vitamin B12 or folate supplements, since they change the picture

What MCH Cannot Tell You
MCH describes your red blood cells. It does not name the reason they look that way.
A low MCH does not confirm iron deficiency, and it cannot separate iron deficiency from thalassaemia trait on its own. That separation usually needs iron studies such as ferritin, and sometimes haemoglobin electrophoresis. Starting iron because one index looked low is a common mistake, and in someone with thalassaemia trait rather than iron deficiency it does no good.
