What is the Thyroid Function Test (T3,T4,TSH) test?

A thyroid test checks how well the small butterfly shaped gland in the front of your neck is working. That gland makes two hormones, T4 and T3, and the National Institute of Diabetes and Digestive and Kidney Diseases notes that these hormones control how your body uses energy, so they affect nearly every organ including your heart.

The standard panel measures three things from one blood sample: T3, T4 and TSH. Together they show whether your thyroid is underactive, overactive, or working normally.

TFT, Thyroid Profile and Thyroid Function Test: The Same Test

If your slip says TFT, TFTs, thyroid profile, thyroid panel or thyroid function test, it is one test. Pakistani labs use all of these names for the same T3, T4 and TSH panel, and many people search for the abbreviation rather than the full name because that is what the doctor wrote.

Some labs also number their thyroid panels, which causes confusion. A numbered panel usually separates the total hormone version from the free hormone version, so check which three markers are actually listed rather than trusting the number.

What the Test Measures

MarkerWhat it isWhy it matters
TSHThyroid stimulating hormone, made in the pituitary glandTells the thyroid how much T4 and T3 to make, and it is what doctors check first
T4Thyroxine, the main hormone the thyroid releasesHigh may mean an overactive thyroid, low may mean an underactive one
T3Triiodothyronine, the more active hormoneUseful when T4 looks normal but an overactive thyroid is still suspected

Reading Your Result: TSH Comes First

TSH is the number your doctor looks at first, and it moves in the opposite direction to what most people expect.

TSH resultWhat it most often means
High TSHHypothyroidism, an underactive thyroid. The gland is not making enough, so the pituitary keeps releasing TSH
Low TSHHyperthyroidism, an overactive thyroid. The gland is making too much, so the pituitary stops releasing TSH
Normal TSHThyroid function is usually normal, though your doctor reads it with your symptoms

NIDDK is clear that if the TSH result is not normal, you need at least one more test to find the cause. That is exactly why the panel measures T4 and T3 alongside it rather than TSH on its own.

What Can Shift Your Result

Reference ranges for T3, T4 and TSH differ between laboratories and between testing methods, so read the range printed on your own report rather than comparing against a number found online.

SituationEffect on the result
PregnancyThyroid hormone levels read higher
Oral contraceptivesThyroid hormone levels read higher
Severe illnessCan lower T4
CorticosteroidsCan lower T4

NIDDK explains why. These situations change the amount of protein in your blood that binds to T4, and bound T4 is held in reserve rather than being available to your tissues. Free T4 is not attached to those proteins, which is why many doctors prefer to measure it when any of the above apply.

Who Should Get Tested and When

Thyroid problems build slowly and the early signs are easy to blame on something else. Ask your doctor about testing if you have any of the following.

  • Unexplained weight gain or weight loss
  • Feeling cold or feeling hot when others around you do not
  • Tiredness that does not lift, or trouble sleeping
  • A racing or irregular heartbeat
  • Hair thinning, dry skin or changes in your periods
  • Swelling or a lump in the front of your neck
  • A family history of thyroid disease

Testing is also common in pregnancy and when starting or adjusting thyroid medicine, on whatever schedule your doctor sets.

How to Prepare

  • No fasting is needed for a thyroid function test
  • Eat and drink normally before the sample
  • Tell your doctor if you are pregnant or taking oral contraceptives, since both lift the readings
  • Tell your doctor about steroids, biotin supplements and any thyroid medicine you take
  • If you are already on thyroid medicine, ask your doctor when to take your dose on test day

What Happens During the Test

  • A trained phlebotomist draws a small blood sample from a vein in your arm
  • The draw takes under a minute
  • You may feel a brief sting and see a small bruise afterwards
  • You can eat and drink normally straight away
  • Your Marham Lab report is ready the same day and reaches you online

Conditions It Can Help Detect

Thyroid blood tests help doctors diagnose hypothyroidism and hyperthyroidism, and find the cause behind them. NIDDK lists Hashimoto disease as the most common cause of an underactive thyroid and Graves disease as the most common cause of an overactive one, both of which are autoimmune conditions.

If your results point that way, your doctor may add a thyroid antibody test. Antibodies are made when the immune system attacks the thyroid by mistake, and measuring them helps separate an autoimmune cause from other explanations. Anti TPO is available at Marham Lab for PKR 1,190. Imaging such as an ultrasound is used for lumps in the neck rather than for checking function.

Final Thoughts

Thyroid symptoms are vague enough that people live with them for years before testing. If you have been tired, gaining or losing weight without reason, or feeling the cold more than everyone else, one blood test settles the question. Take the report to a doctor rather than reading it alone, because TSH, T4 and T3 only make sense together, and thyroid medicine is adjusted on the pattern rather than on a single number.

Frequently asked questions

Yes. TFT, TFTs, thyroid profile, thyroid panel and thyroid function test all describe the same blood test. It measures T3, T4 and TSH from one sample. Pakistani labs use these names interchangeably, so if any of them appears on your slip, this is the test your doctor wants.

At Marham Lab the thyroid function test covering T3, T4 and TSH is PKR 2,730, reduced from PKR 3,900, and the rate is the same in every city. Home sample collection is currently available in Lahore only, so patients elsewhere book a lab visit. Prices at other laboratories vary widely, so confirm the rate when you book.

You can. At Marham Lab TSH on its own is PKR 1,190, and T3 or T4 alone are PKR 1,050 each. Doctors often start with TSH, so a single TSH is reasonable when that is what was asked for. Booking all three separately comes to PKR 3,290, which is more than the PKR 2,730 panel, so if your doctor wants the full picture the panel is the better value. NIDDK also notes that an abnormal TSH always needs at least one further test to find the cause.

The standard panel measures total T3 and T4, which includes the hormone bound to proteins in your blood. The free panel measures only the portion not bound to those proteins, the part actually available to your tissues. NIDDK notes that pregnancy, oral contraceptives, severe illness and corticosteroids all change binding protein levels, which is why many doctors prefer free hormone measurements in those situations. At Marham Lab the standard panel is PKR 2,730 and the free panel is PKR 3,500. Follow whichever your doctor requested.
