About Poliomyelitis

Poliomyelitis, commonly known as polio, is a highly infectious viral disease that primarily affects young children. The poliovirus enters the body through the mouth, multiplies in the intestines, and can invade the nervous system which causes paralysis.

In many cases, the infection is mild or asymptomatic, but in severe cases, it can lead to irreversible muscle weakness or paralysis, especially in the legs. Although there is no cure for poliomyelitis, vaccination can effectively prevent it. Global immunization programs have successfully reduced polio cases worldwide.

Poliomyelitis Symptoms

Symptoms of poliomyelitis depend on the type and severity of infection. Early signs may include:

  • Fever
  • Sore throat
  • Headache
  • Fatigue
  • Weakness
  • Nausea
  • Vomiting
  • Neck and back stiffness
  • Pain in limbs or muscles

In severe cases, symptoms may include:

  • Sudden onset of paralysis (usually in the legs)
  • Difficulty breathing or swallowing
  • Loss of reflexes
  • Muscle tenderness and spasms

Poliomyelitis Causes

Poliomyelitis is caused by the poliovirus, which spreads mainly through the fecal-oral route. The main causes and transmission methods include:

  • Contact with the feces or saliva of an infected person
  • Consumption of contaminated food or water
  • Poor hygiene and sanitation practices
  • Crowded living conditions
  • Weakened immune system
  • Lack of polio vaccination

Poliomyelitis Risk Factors

Certain factors increase the risk of developing poliomyelitis, including:

  • Incomplete or no vaccination against polio
  • Living in or traveling to areas where polio is still present
  • Poor sanitation and hygiene
  • Weak immune system
  • Children under the age of five
  • Crowded living conditions
  • Malnutrition

Poliomyelitis Prevention

Poliomyelitis can be effectively prevented through vaccination and hygiene measures:

  • Complete all recommended doses of the polio vaccine (OPV or IPV)
  • Maintain good personal hygiene and handwashing
  • Drink clean and safe water
  • Avoid contact with infected individuals
  • Participate in national immunization campaigns
  • Ensure proper sanitation and waste disposal

Poliomyelitis Types

There are three main types of poliomyelitis, depending on the severity of symptoms and nervous system involvement:

Abortive Poliomyelitis:

This is a mild form of infection with flu-like symptoms such as fever, sore throat, headache, and fatigue. It does not affect the nervous system.

Non-Paralytic Poliomyelitis:

In this type, the virus affects the nervous system but does not cause paralysis. Symptoms may include neck stiffness, back pain, and muscle weakness.

Paralytic Poliomyelitis:

This is the most severe form that causes paralysis due to damage to the spinal cord and brain. It may lead to loss of muscle function, respiratory failure, or even death in critical cases.

Poliomyelitis Diagnosis

Doctors diagnose poliomyelitis by evaluating symptoms, medical history, and performing laboratory tests. Diagnostic methods include:

  • Stool or throat swab tests to detect poliovirus
  • Cerebrospinal fluid (CSF) analysis
  • Blood tests for antibodies against the poliovirus
  • Neurological examination to check muscle strength and reflexes
  • MRI scans to assess damage to the spinal cord in severe cases

Poliomyelitis Treatment

There is no specific cure for poliomyelitis; treatment focuses on managing symptoms and preventing complications. Following approaches are mostly used:

Supportive Care:

  • Adequate rest and hydration
  • Pain-relief medications for muscle pain and cramps
  • Use of braces, wheelchairs, or other mobility aids

Physical Therapy:

  • Rehabilitation exercises to improve mobility and prevent muscle contractures
  • Strength training to recover partial muscle movement
  • Occupational therapy to assist with daily activities

Respiratory Support:

In severe cases, mechanical ventilation may be needed if breathing muscles are affected.

Nutritional Support:

A balanced diet to promote recovery and strengthen the immune system

Psychological Support:

Counseling and emotional support for patients and their families to cope with the effects of paralysis

Poliomyelitis Complications

If not treated or prevented, poliomyelitis may lead to serious complications such as:

  • Permanent paralysis of limbs
  • Breathing difficulties due to respiratory muscle damage
  • Post-polio syndrome (recurrence of symptoms after years)
  • Deformities in bones and joints
  • Difficulty in swallowing and speaking
  • Muscle wasting and weakness
  • Emotional and psychological challenges
