I am suffering from low hb issue since 2015 every year my Hb became low. Now a days I am suffering with same issue and my treatment is ongoing here in Riyadh under Hematologist but when he started my treatment my Hb was 8.3 and he recommended Feriject intraven 50MG/ML two injection for two week one dose for one week at that time my Ferriten was 288 and ESR 100 but after two weeks CBC report reflect my Hb became 6.63 ,Ferriten 587,ESR 135,MCV 92.6. Ig G quant 21.3 and Free light chain type Kappa is19.8 and Lambda is 41.7 and B12 is 24.6. All reports are attached, I am very much veried.Now he recommnded agian Ferijet (iron injection) for 2 weeks and Neurobin 3ml intramus for 5 days. Kindly check my reports and guide me.
AOA thanks for using Marham for health consultation actually the problem is macrocytic anemia with low b12 levels for this you required inj neurobion 1amp daily for 7 days then weekly for 1 month and also RBC Folate levels. Presently u do not required ferinject (iron therapy) as your iron stores (ferritin) are already sufficient.

Any Iron studies done.??. What about Serum Folate levels.?.
How are your die try habits. Any History of long term loose motion .?Any pain in abdomen
You need see medical specialist first as you are not yet diagnosed, I mean the cause of your deficiency need to be known first .why is your ESR high ,this is important to know too .any history of loose motions or constipation, weight loss .

Also Your BP pulse check your thyroid functions and lipid profile too .

Doctor sahiba.My BP pulse remain 100 pluse. My TSH value 0.096, FT4 18.42, FT3 is 2.98.TSH receptor abs is less than 1.

My ultrasound report for thyroid gland. Shows , both thyroid gland lobes are normal in size,Rt lobes shows a small heterogeneous nodule measurea about 4×3mm and the Lt. Lobe shows a small anechoic cyst measures abt 1.2×0.2 cm.
The isthmus is seen free.
Normal structures around.
No retro sternal extension.
Normal vascularity of both lobes.
Bilateral enlarged cervical lymph nodes the largest on the right side measuring about 1 cm and the largest on the left measuring 1.5 cm with preserved fatty hilum

Actually thyroid doctor said ,1st need to maintain your hb value which is right now 6.63 then after I will give you medicine for thyroid after checking nuclear thyroid test. These all report are fresh of current month and also in Nov.

Get injection B 12 and go for endocrinologist then

Kindly advise me, what will be the causes of high ESR as in my case its value is 135.

As reset test reports, my tsh became 0.0003 and T4 free is 19.8 while Hb value is 6.4

Your probable diagnosis is multiple myeloma which is causing both anemia and raised ESR

Salam Doctor Sb, Did you check my Free Light chains & Immunifixcation Reports. Kappa/Lambda Ratio which is in the normal range that is 0.480. I don't have pain in my bones.

Thanks for reply .Doctor sb I showed my concerns with my Doctor here in Riyadh about ferijet drips as my ferritin is 587 but he said this is due to ESR which is 135 and u need both fetijet and neurobin. I shared my Free light chains report also
