Talk to Pulmonologist / Lung on Smokers Cough

My cousin is having a serious problem, he is a smoker since 4 years, his coughing is so rough and it gives him pain in lungs, he has never been to a doctor he's scared what if he gets to know about any other diseases in him. I really want to ask what medicines or treatment is required. Thankyou
