Talk to Urologist on Testicles & Penis Issue

Salam, i am from karachi, i am having this skin issue from last 10 years almost, testicle ki skin mey cuts lag jatey hain, bht zyada itching hoti hai, jese rough skin hati hai wese hi white color ki rough skin jharti hai skin ke uper se, pain nahi hota, but haan jahan zyada cuts hote hain wahan halka sa skin ke uper pain hota hai, aur yeh porey testicle pe nahi hai sirf right waley pe hai aur neche ki taraf, irritation bohat hoti hai, i used oil to remove it and warm water to rub and remove when i am taking shower. jab rub kar ke remove ho jata hai to skin alag se feel hoti hai bilkul chikni type ki. please help me. What step should i take. Secondly recommend me good sexologist in karachi but please not urologist. I am having a timing issue less than 1 min and as well as girth issue which is not enough thick.
