Asking for Self, Female, 23 years old, karachi
had issue of perianal pain then my ischiorectal abscess burst. Took antibiotics but still puss/blood was leaking from the wound. Took this test. Now my question is that my wound is on my bum near anus about 6/7 cm away from anus and not on the natal cleft but acc to internet pilonidal sinus occurs mostly in male and also on natal cleft. So being a female why i got it and that too on my bum?? I mean what could be the reason for it??

This is perianal sinus, occurs due to infection of anal glands, underlying cause is constipation, diabetes mellitus should also be ruled out

this seems to be PERI ANAL SINUS and not the PILONIDAL SINUS
You need a proper physical examination by a surgeon to determine the complexity of the disease and discuss the treatment options.

Thanks but my doctor said that only one treatment is available for sinus and that is completely opening of sinus tract and then open wound left for healing. Are there any other methods?

Yes laser is the growing modality of sinus treatment and have very good results,need to review mri films clinical examination with necessary work up

there is no hard and fast rules in medical line.
pilondal sinus may occur in female.
need proper examination and treatment accordingly and the main stay of treatment is surgery.
you need examination under anaesthesia
Pilonidal sinus or complex perianal fistula need examination
it could be perianal fistula due to perianal abscess need sonogram ana examination under anesthesia and proceed according to per examination findings.
need to be examined under anesthesia by general surgeon
according to your MRI it's a sinus at buttock region
reason may be abscess as you told in your history
treatment will be complete excision followed by primary/delayed closure

it's not a pilonidal sinus , which is classically in natal cleft, it's a sinus ,which is a blind ending tract secondary to ischio rectal abscess. further details possible after examination.

It's a sinus tract after the abscess that has burst open. It needs proper drainage in order to heal
You might have perianal fistula or sinus. This needs exploration ( surgery). Did any urine, stool or something like gas came out thru the external opening? It can explain its communication with anal canal or vagina otherwise tract is extending in perineal muscles. This is not pilonidal sinus.

Got my Fistulogram test and it showed that's it a sinus with no communication with rectum or anal canal.
I want to know which surgery would be best?? And acc to reports is this a large sinus tract or a normal one?

It is a complicated surgery and needs to excise whole sinus tract. Sometimes, recurrence can be if not completely excised the tract. During surgery, perineal muscles can be damaged. It is the per operative decision, to which extent we can go for surgery.

This is pilonidal sinus and by surgery it will be cured but still there will be chances of recurrence
Even it can be completely cured with medicine and there will be no chances of recurrence because in it's medication its cause of such sinus will be ruled out and it's basic reason will be cured so that in future you will not suffer from such a condition again
It's up to you either you choose surgery or medication but if you choose medication then it will be safe and you will recover completely
