Asking for Self, Female, 26 years old, Lahore

As salaam o alaikum Doctor I just found out that I'm 6 weeks pregnant with my second, first one is 9 months old and I had a c section. I had PCOS and I was on xormet when I conceived my first baby. After delivery I got hypothyroidism and right now I'm taking thyronom 25 MG once, 5 days a week and two tablets on Saturday and Sundays. My doctor asked me to visit gynecologist as soon as I get pregnant but right now due to COVID-19 I can't go out Can you guide me what am I supposed to do, what medications or US and what about thyronom intake. Thank you
