Thanks for asking. I think we should know first about the history of the patient and it should include detailed examination. As ultrasound has already been done and fluid is also aspirated from the abdomen. We should wait for the reports of aspiration. I think if reports are not conclusive then we have to go for the CT ABDOMEN AND PELVIS WITH CONTRAST. I wish there would be ESR in the blood test which is missing in CBC. CA 125 is also required.....
It depends upon the report of ascitic fluid analysis.

it definitely will be treated surgically , but pathology report to rule out cancer or any TB abd should also be ruled out along
You have options of US guided biopsy but the problem with o mental biopsy is that it may not always be positive. I can do laparoscopic biopsy of omentum(key hole)
There is an acoustic shadow in the gall bladder (pitta) which can either be stones or grit . depending upon what are your complaints you need to get an examination and discuss in details what are you suffering from for an appropriate plan of treatment
